DYNAMICS OF THE DENTAL STATUS, THE ANALGESIA/NOCICEPTION INDEX AND THE CONTENT OF HYDROGEN SULPHIDE, INDICATORS OF NITRIC OXIDE METABOLISM IN THE ORAL FLUID OF PATIENTS UNDER THE CONDITIONS OF DENTITIONS CORRECTION

Abstract
The results of dynamic examination of the dental status, the
Analgesia Nociception Index (ANI), the content of hydrogen
sulfide (H2S) and the indicators of nitric oxide metabolism
(NO) in the oral fluid under conditions of dentitions correction
are presented in the article. The evaluation of nociception/
antinociception balance was performed by using ANI-Monitor
apparatus (MetroDolores, France). Patients without concomitant
pathology with partial loss of teeth under conditions of
defects compensation with bridging and partial removable
prostheses (primary and re-prosthetics), with complete loss of
dentition with correction with complete removable prostheses
(primary and re-prosthetics) were examined. The examination
was performed before prosthesis and also after 24 h, on the
7th and 30th day after dentition correction. It was established
that in the structure of periodontal tissue morbidity at a young
age the gingivitis was predominant, in some cases generalized
periodontitis of varying severity, the periodontal disease was
often revealed with the age. In examined persons who used
prostheses the signs of trauma of the prosthetic bed mucosa
was observed. It was found that ANI values depend on the
state of oral mucosa. The most pronounced changes of the
index were established after the primary prosthesis and under
the condition of correction of the dentition with complete
removable laminar prostheses. In such patients, the adaptation
processes were the longest (to 30 days) and were accompanied
by a steady increase of H2S, sum of NO metabolites (NO2
-
+
NO3
-
) and peroxynitrite contents in the oral fluid. Thus, the
dynamics of changes in ANI and H2S content in the oral fluid
reflects the course of adaptive-compensatory reactions after
the conditions of dentitions correction of dental rows. Indicators
of accelerated metabolism of NO (peroxynitrite, sum of
NO2
-
+ NO3
-
) can be regarded as predictors of disorders of
dental status, which allow to estimate the probability of pain
and formation of trophic disorders in preclinical stages of
disorders. The ANI value reflects the degree of nociception
and may act as a marker of adaptation to prostheses.
